Role:
As a Licensed Practical Nurse (LPN) at Hampton Nursing and Rehab, you will play a vital role in our team's efforts to provide exceptional care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medications, and perform vital signs. Your exceptional communication skills and passion for patient-centered care will ensure that our residents receive the highest level of care and attention.
Responsibilities:
Provide direct patient care under the supervision of the Registered Nurse, including administering medications, performing vital signs, and providing wound care Assist with daily living activities, such as bathing, dressing, and feeding Develop and implement individualized care plans, in collaboration with the interdisciplinary team Monitor and report any changes in patient condition to the Registered Nurse Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Requirements:
Current LPN licensure in the state of Michigan Graduation from an accredited LPN program Ability to lift, transfer, and reposition patients with assistance as needed Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment
