$45- 50 per visit About the Role Guardian Angel Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing home health team in the Grand Rapids area.

- HCHB ) Supportive clinical leadership Opportunities for professional growth and advancement Mileage reimbursement Responsibilities Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician Monitor patient condition, vital signs, and overall health status Administer medications and treatments as ordered and within the LPN scope of practice Accurately document all visits and care provided in Homecare Homebase (HCHB) Communicate changes in patient condition to the supervising RN and physician Educate patients and families on medications, disease management, and the plan of care Ensure compliance with all federal, state, and agency regulations Collaborate with the interdisciplinary team to achieve optimal patient outcomes Requirements Active Michigan Licensed Practical Nurse (LPN) license in good standing Graduate of an accredited practical nursing program Current BLS/CPR Certification Valid driver's license, reliable transportation, and current auto insurance Strong clinical, communication, and documentation skills Ability to work independently in a home health setting Preferred Qualifications Home health or hospice experience Experience with Homecare Homebase (HCHB) or other electronic medical record systems Wound care experience Bilingual candidates are encouraged to apply Why Join Us?
