Hospice On Call RN or LPN 7on/7off
Location:
Lansing, Mason, Dewitt, and surrounding areas
Job Type:
[Full-Time] We're hiring a Hospice On Call RN or LPN to provide skilled nursing care and manage patients at the end of life.
Key Responsibilities:
Provides after-hours, weekend, and holiday nursing coverage for hospice patients during assigned 7-day on-call rotations. Responds promptly to patient, family, and facility calls regarding symptom management, changes in condition, and end-of-life care needs. Performs urgent nursing visits to assess patient status, manage symptoms, and prevent unnecessary hospitalizations. Triages incoming calls and prioritizes patient needs based on clinical urgency and hospice eligibility criteria. Supports patients through crisis situations, actively working to maintain comfort and dignity in the home, facility, or inpatient setting. Provides death visits (RNs), pronounces death (RNs) when permitted by state regulations, and offers immediate bereavement support to families Must be okay with traveling throughout service area
Qualifications:
Active Michigan RN or LPN license 1+ year nursing experience (hospice) Strong communication and organization skills Valid driver's license and reliable transportation HCHB experience preferred
