No County Seneca Street Address Dick Van Dyke Addiction Treatment Center 1330 County Road 132 City Ovid State NY Zip Code 14521 Duties Description Under the supervision of a Registered Nurse, incumbents serve as members of a multidisciplinary team performing a variety of nursing and client care activities designed to ensure that clients receive all necessary care. Responsibilities of this position include, but are not limited to:

• Medication administration and specimen collection;
• Attentiveness to patients' medical and other needs;
• Provision of nursing treatments;
• Gather patient measurements, signs and symptoms;
• Conduct building checks for safety hazards and ensuring patient safety;
• Crisis intervention;
• Complete documentation in patient electronic medical records, as well as staff communication logs and other required forms as needed;
• File patient record forms in the appropriate location within the record;
• Performs other related duties as assigned.

Minimum Qualifications A license to practice as a practical nurse in New York State. Candidate must be eligible and maintain eligibility for full and unconditional participation in the Medicaid and Medicare programs. Failure to maintain licensure, certification and Medicaid/Medicare eligibility will result in the termination of employment. Dick Van Dyke ATC is a tobacco-free facility.
