Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our long-term care team. Responsibilities Deliver comprehensive nursing care to residents, including medication administration, wound care, IV infusions, and vital signs monitoring. Conduct thorough patient assessments and develop individualized care plans in collaboration with the healthcare team. Manage acute pain and assist with complex medical procedures such as catheterization, tube feeding, and sterile dressing changes. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient information accurately and maintain compliance with privacy regulations such as HIPAA. Provide specialized care for residents with dementia or memory impairments, including Alzheimer's and behavioral health management. Assist in triage assessments for residents presenting with urgent health concerns or infectious disease symptoms, ensuring timely intervention. Collaborate effectively with physicians, therapists, caregivers, and families to coordinate ongoing care and support resident needs within long-term care regulations. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the state of employment. Proven experience working in long-term care facilities such as skilled nursing facilities or assisted living communities. Demonstrated proficiency with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; experience with Cerner preferred. Strong knowledge of medical terminology, anatomy, physiology, and clinical confidentiality policies. Experience managing complex patient cases including IV infusions, catheterization, pain management, and infection control practices. Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS). Excellent communication skills for caregiver collaboration and client interaction across diverse populations including geriatric patients and individuals with disabilities. Pay:

$1,620.00 - $1,728.00 per hour Expected hours: No less than 36.0 per week
