Job Description

Job Summary We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. In this vital role, you will provide comprehensive nursing care to residents in a long-term care facility, ensuring their health, safety, and comfort are prioritized. The ideal candidate will possess strong clinical skills, experience working with geriatric and disabled populations, and a commitment to delivering compassionate patient-centered care. This position offers an opportunity to make a meaningful difference in the lives of our residents through skilled nursing practices and collaborative teamwork. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation. Administer medications and injections accurately following prescribed protocols, including IV infusions when necessary. Monitor residents for signs of acute pain, infection, or deterioration; respond promptly with appropriate interventions. Manage complex care needs such as dementia, Alzheimer's disease, and memory care requirements. Document all patient interactions and treatments accurately within Electronic Health Record (EHR) systems while maintaining strict compliance with HIPAA regulations. Assist with diagnostic evaluations, specimen collection, and processing as required. Provide wound care, catheterization, tube feeding management, and ventilator support under supervision. Collaborate with interdisciplinary teams including physicians, therapists, and case managers to develop individualized care plans. Educate residents and their families on health management strategies and medication administration. Ensure compliance with long-term care regulations and facility policies to promote safe and effective care delivery.

$40.00 per hour
