We are seeking a Licensed Practical Nurse (LPN) to join a home health team serving patients throughout the Chisago City, Minnesota area . This full-time opportunity allows you to provide individualized nursing care directly in patients' homes while working alongside a supportive and experienced clinical team. Schedule & Compensation Full-Time Monday-Friday, 8:00 AM-5:00 PM $66,000-$74,000 annually Compensation determined by nursing experience and a point-based system Participation in an established on-call rotation is required Local travel to patient homes within the assigned territory What You'll Be Doing In this role, you will help patients safely manage their healthcare needs while remaining in the comfort of their own homes.

Responsibilities may include:

Providing skilled nursing visits according to physician orders and individualized plans of care Assessing patients and monitoring changes in their condition Administering prescribed medications, treatments, and nursing interventions Educating patients and caregivers regarding medications, diagnoses, treatments, and ongoing care needs Completing accurate and timely clinical documentation Communicating changes in patient status to the appropriate members of the care team Collaborating with physicians, therapists, case managers, and other healthcare professionals Supporting patient safety, comfort, independence, and overall quality of life Participating in the required on-call rotation and responding to patient needs as appropriate Qualifications We are looking for an LPN who can combine strong clinical skills with excellent communication and time management.
