POSITION SUMMARY
We are seeking a compassionate, organized, and clinically knowledgeable Licensed Practical Nurse (LPN) / Admissions Coordinator to join our residential substance use disorder treatment program. This position combines nursing responsibilities, admissions coordination, and medication-related support within a Minnesota DHS 245G-licensed residential treatment setting. The LPN / Admissions Coordinator serves as an important first point of contact for individuals seeking treatment and works collaboratively with the clinical and residential teams to ensure that prospective clients are appropriately screened, safely admitted, and connected with the services identified in their individualized treatment plan. The ideal candidate will demonstrate strong clinical skills, excellent communication skills, attention to detail, and a commitment to trauma-informed, person-centered, culturally responsive substance use disorder treatment.
ESSENTIAL RESPONSIBILITIES
Coordinate and oversee the residential admissions process from initial referral through admission. Gather and review referral information, including: Substance use history Current medications Assist with admission screening. Coordinate with referral sources, hospitals, detoxification programs, counties, courts, healthcare providers, and other professionals as appropriate and authorized. Obtain required releases of information and ensure appropriate confidentiality practices. Coordinate admission dates, transportation arrangements, medication information, and other admission logistics. Ensure required admission paperwork is completed accurately and timely. Ensure prospective clients understand program expectations, client rights, responsibilities, policies, and procedures. Provide nursing services within the scope of the LPN license and applicable Minnesota requirements. Follow program policies and procedures for medication storage, administration, documentation, and monitoring. Communicate medication concerns, adverse reactions, changes in condition, and other health-related issues to the appropriate healthcare professional. Coordinate medical appointments and healthcare follow-up when assigned. Maintain admissions documentation in accordance with Minnesota
DHS 245G
requirements and agency policy. Assist with preparation for DHS licensing reviews, audits, and corrective action activities. Maintain confidentiality in accordance with applicable federal and state laws, including HIPAA and 42 CFR Part 2. Serve as a liaison between Admissions, Nursing, Clinical, Residential, and Administrative departments. Participate in staff meetings, case consultation, treatment coordination, and quality improvement activities. Communicate pertinent admission and health information to the treatment team while maintaining appropriate confidentiality. Support a safe, respectful, recovery-oriented therapeutic environment. Coordinate with medical staff regarding admission-related medical or health concerns within the LPN's scope of practice. Assist with development and implementation of admissions procedures, forms, checklists, and quality improvement processes. Perform other duties consistent with the position and program needs.
REQUIRED QUALIFICATIONS
Current, unrestricted Minnesota LPN license in good standing. Experience working with individuals with substance use disorders preferred. Experience in residential treatment, behavioral health, detoxification, corrections, healthcare, or another related setting preferred. Knowledge of medication management. Strong understanding of confidentiality and professional boundaries. Excellent written and verbal communication skills. Strong organizational and time-management skills. Ability to work independently while functioning effectively as part of a multidisciplinary team. Ability to maintain accurate and timely documentation. Ability to demonstrate sound judgment regarding client safety and appropriateness for admission.
PREFERRED QUALIFICATIONS
Previous admissions or intake experience in a substance use disorder treatment program. Knowledge of Minnesota 245G requirements. Experience with electronic health records (EHR). Experience coordinating with counties, hospitals, detoxification programs, healthcare providers, and referral agencies. Experience with medication-assisted treatment (MAT/MOUD). Experience working with co-occurring mental health and substance use disorders. Experience in a trauma-informed and culturally responsive treatment environment. Previous supervisory, leadership, or program coordination experience.
