Job Description
Licensed Practical Nurse (LPN) - Evening Shift Position Summary The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care, medication administration, clinical oversight, and leadership of caregiving staff to ensure the health, safety, and well-being of residents. The Evening LPN serves as the clinical leader during evening hours, responding to changes in resident condition, coordinating emergency situations, and ensuring all resident care is provided in accordance with state regulations, physician orders, and Trouvaille policies and procedures. Essential Responsibilities Resident Care Provide skilled nursing care to residents in accordance with physician orders and individualized service plans. Administer medications, treatments, and delegated nursing procedures safely and accurately. Assess residents for changes in physical, mental, or emotional condition and document findings appropriately. Respond promptly to resident emergencies, falls, illnesses, and other urgent situations. Communicate significant changes in resident condition to physicians, on-call providers and leadership as appropriate. Monitor residents for pain, infections, skin integrity concerns, behavioral changes, and other health-related issues. Assist caregivers with resident care, including transfers, personal cares, and repositioning. Ensure resident rights, dignity, privacy, and independence are maintained at all times. Medication & Clinical Responsibilities Administer and document medications in accordance with physician orders and medication administration policies. Complete narcotic counts and medication cart audits as required. Ensure medications, treatment supplies, and emergency equipment are properly stocked and secured. Complete required nursing documentation accurately and in a timely manner. Monitor and document vital signs, blood glucose levels, weights, and other clinical observations as ordered. Follow infection prevention and control procedures at all times. Ensure compliance with state regulations, assisted living licensing requirements, and company policies. Supervisory Responsibilities Serve as the Evening clinical lead for caregivers Delegate nursing tasks appropriately while ensuring competency and accountability. Provide guidance, coaching, and support to overnight staff. Monitor staff performance and ensure assigned duties are completed accurately and efficiently. Assist with orientation and training of new staff as assigned. Address immediate staffing concerns, workflow issues, and resident care concerns during the shift. Communicate significant incidents, concerns, or policy violations to the Director of Nursing or Administrator. Foster teamwork, professionalism, accountability, and a positive work environment. Safety & Quality Assurance Maintain a safe, clean, and secure environment for residents, visitors, and staff. Ensure all safety checks, security rounds, and overnight responsibilities are completed. Complete incident reports and required documentation following falls, injuries, medication errors, or unusual occurrences. Participate in emergency preparedness and respond appropriately during emergencies. Report equipment malfunctions, maintenance concerns, and safety hazards promptly. Ensure confidentiality of resident information in accordance with HIPAA regulations. Communication Communicate effectively with residents, families, physicians, pharmacies, hospice providers, and other healthcare professionals. Provide thorough shift reports to incoming nursing staff. Collaborate with interdisciplinary team members to ensure continuity of care. Document resident care clearly, accurately, and objectively. Qualifications Current Licensed Practical Nurse (LPN) license in the State of Minnesota in good standing. Current CPR certification (or ability to obtain upon hire). Previous experience in assisted living, memory care, long-term care, or geriatric nursing preferred. Experience supervising or leading staff is preferred. Knowledge of dementia care and person-centered care practices is preferred. Ability to exercise sound clinical judgment and respond effectively in emergency situations. Ability to safely assist with resident transfers and repositioning using proper body mechanics and equipment. Ability to lift up to 50 pounds and assist with heavier transfers as part of a team.
