Windy Acres is hiring a Nurse Director that is able to wear many hats for our small facility of upto 15 residents age 55+ and upto 20 employees. An active current MN nursing license is required . LALD preferred; but willing to assist in obtaining via coordination of training and covering costs through BELTSS upon hire. Willingness to obtain is required . Background check required through DHS, and BELTSS for LALD portion. The salary range is 59k-80k based on current license and background and training needed. The position is full-time and on-site, but has flexibility. On-call rotation requirement includes every other weekend with phone reimbursement or phone provided. We do not have a comprehensive benefits package, as we are small; however we do offer flexibility, 401k, meals on shift, uniform reimbursement, and PTO. Knowledge of Chapter 144G, customized living and Assisted living with Dementia care laws, statutes and guidelines are helpful but not mandatory. Knowledge or interest in geriatric population also helpful, but not mandatory. The LALD is responsible for day to day function of the entire facility and all aspects related from coordinating maintenance to writing policies to serving a meal to shoveling a sidewalk to calling a clinic to training staff to communicating with families to completing an admission and everything in between. The LALD is the primary contact for state surveyors and the go to for answers related to the facility and its operation under licensing requirements. The nurse is responsible for the day to day resident care, assessments, admissions, delivery of care, scheduling of staff, staff hiring and training, and answering care related questions 24/7. Critical thinking and multiple care team member communications from doctors to case manager to county waivers to employees to family member to guardians is necessary. An RN will have the title of clinical nurse supervisor and need to be available for state surveys. This position combines both roles for an all encompassing unique role for a small facility and has a responsibility list including, but not limited to: Facility management: day to day operations, maintenance coordination, maintain compliance with MDH/DHS requirements and standards, maintain and implementation of policy and procedures, maintenance and oversight of administrative records, manage grievance, manage and coordinate meetings, manage emergency preparedness policy and procedures and compliance, coordinate quality assurance activities, conduct routine audits of records and correct as needed, monitor and implement licensing changes, oversight of all facility departments. On-site for all state surveys.
Employee management:
Interview, hire, train and maintain records relating to employees, monitor employee compliance and ongoing training per requirements, monitor for employee qualifications, background, conducting file audits and correction as needed, assist with disciplinary action and terminations, assist in administrative paperwork. Answer care related questions in real time 24/7, respond to none urgent staff questions and needs within business hours.
Resident management:
Assessments, admissions, care coordination, and delivery of services while protecting rights, and providing cares with dignity and respect, focus on patient centered care, manage staffing ratios, manage incidents and reporting requirements, conducting audits and correcting as needed, safe discharge planning where applicable, complete administrative paperwork, communication with hospitals, responsible parties, guardians, case managers, staff, etc. Candidates should have an exceptional ability to communicate across a wide variety of individuals including dementia residents, overwhelmed families, and stressed staff. Ideal candidate will be highly motivated and able to work independently and have pride in the work they do for the people they do it for. We serve a population that needs us the most, and we do so with an exceptional level and standard of care expectation of our staff. Although this position is all encompassing there are supports to the position such as admin assistants, involved owner (nurse), and the facility is and has been fully operational for over 10years through multiple license changes and surveys.
