Job Responsibilities Provide skilled nursing visits to patients in their homes in accordance with physician orders and care plans. Perform patient assessments, monitor health status, and document findings accurately. Administer medications, treatments, and nursing interventions as prescribed. Educate patients and families on disease management, medications, treatments, and home care needs. Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams. Complete routine home health visits while meeting productivity expectations of 32 points per week (routine visit = 0.9 points). Maintain timely and accurate documentation using Homecare Homebase (HCHB) . Monitor changes in patient condition and report significant findings to the appropriate healthcare providers. Ensure compliance with agency policies, state regulations, and home health standards. Provide coverage in additional territories within approximately a 60-mile radius of the facility as needed. Requirements Active and unrestricted LVN/LPN license . Recent Home Health experience (2024-2025) required. Homecare Homebase (HCHB) experience strongly preferred. Candidates without HCHB experience may be considered if willing to complete training. Strong assessment, clinical documentation, and patient education skills. Valid driver's license, reliable transportation, and auto insurance. Ability to travel throughout assigned territories and surrounding service areas. Excellent communication, organizational, and time-management skills. Ability to work independently while effectively managing a daily patient schedule.
