Job Description
Job Title:
Licensed Practical Nurse (LPN) |In-Office Primary Care & Triage Company:
Twin Cities Physicians Location:
In-Person |Golden Valley, Minnesota Schedule:
Full-Time | Monday through Friday |No Nights, Weekends, or Holidays Compensation:
$26.75-$29.00 per hour , based on experience How to Apply Qualified candidates should email Tom Lecy at tom.lecy@hhcare.net with a brief summary of your nursing background, any experience you have with telephone or clinical triage, and what interests you about joining Twin Cities Physicians. A Nursing Career Without the Bedside Schedule You became an LPN to help people, use your clinical knowledge, and make a difference in patient care. But being a nurse does not have to mean spending your entire career working nights, weekends, holidays, rotating shifts, or physically demanding bedside positions . At Twin Cities Physicians, we're offering experienced LPNs a different way to use their license. This is a 100% in-person, office-based nursing position at our Golden Valley location where you'll work a consistent Monday through Friday schedule with no nights, weekends, or holidays . Instead of managing a bedside assignment or driving from patient to patient, you'll use your nursing experience to help coordinate care for more than 4,500 seniors living in 350+ senior living communities throughout Minnesota . You'll spend your day triaging clinical concerns, communicating with providers, coordinating patient care, processing orders and medication requests, and helping determine what needs to happen next . No bedside nursing. No field visits. No weekend rotation. No holiday rotation. If you still love being a nurse but are ready for a different environment and a more predictable schedule, this could be the next step you've been looking for. Use Your LPN Experience in a New Way You've developed instincts throughout your nursing career that can't be taught in a textbook. You know when something doesn't sound right. You know which questions to ask when a patient has a change in condition. You understand medications, provider orders, symptoms, documentation, and when something needs to be escalated. Those skills translate incredibly well to clinical triage. A senior living nurse may call because a resident has new symptoms. A pharmacy may need clarification on a medication. A family may have a clinical concern. A provider may need additional information before determining the next step. You'll help gather the clinical picture, identify what matters, communicate with the appropriate provider, and make sure there is follow-through. You're not stepping away from nursing. You're expanding what you can do with your LPN license. What Nursing Could Look Like Monday Through Friday For many nurses, finding a predictable weekday position can mean moving away from patient care entirely. This position gives you another option. You'll continue using your LPN license, clinical judgment, and healthcare experience while developing valuable skills in clinical triage, primary care, geriatrics, medication management, care coordination, and transitions of care . You'll also work alongside physicians, nurse practitioners, physician assistants, nurses, pharmacies, and senior living clinical teams , giving you exposure to a side of healthcare you may not experience in a traditional bedside position. This could be a great fit if you're an experienced LPN who is ready to trade the physical demands and unpredictable scheduling of traditional nursing for a professional, office-based clinical career . Why LPNs Are Interested in This Role Use your LPN license and clinical judgment without traditional bedside nursing Work Monday through Friday with no nights, weekends, or holidays Enjoy a consistent in-person, office-based position in Golden Valley Earn $26.75-$29.00 per hour , based on experience Transition away from the physical demands of bedside or field nursing Build experience in clinical triage, primary care, geriatrics, and care coordination Work directly with physicians, NPs, PAs, nurses, pharmacies, and senior living teams Help coordinate care for more than 4,500 older adults throughout Minnesota Develop skills that can help you continue growing your long-term nursing career What You'll Be Doing You'll become one of the central clinical communication points for our practice, helping determine what needs immediate attention, what needs provider review, and what can be appropriately coordinated through the care team.Responsibilities include:
