Use the Skills You've Worked Hard to Build One patient may need wound care. Another needs medication education after returning from the hospital. Your next patient may have diabetes, CHF, COPD, or another chronic condition requiring skilled monitoring. Later, you notice something has changed with a patient you've seen before. You assess the situation, communicate your findings to the RN Case Manager, and help make sure the concern gets addressed. You're not simply completing a task. You're using your nursing judgment. They're often recovering after hospitalization, learning to manage new diagnoses, dealing with multiple medications, healing wounds, or living with chronic medical conditions. As their LPN Visit Nurse, you'll provide the hands-on skilled care ordered as part of their plan while also paying attention to what you're seeing. What You'll Be Doing Complete skilled nursing visits in patient homes Perform ongoing nursing assessments within LPN scope Administer medications and treatments according to physician orders Provide wound care and other ordered skilled treatments Monitor chronic and medically complex conditions Reinforce disease management and medication education Educate patients, families, and caregivers Recognize and report changes in condition Communicate findings to RN Case Managers and the clinical team Support individualized plans of care Complete accurate and timely clinical documentation Help patients safely recover and remain at home Participate in required on-call and weekend rotations Coming From Long-Term Care, TCU, or Skilled Nursing? What You Bring Required:
Active Minnesota Licensed Practical Nurse (LPN) license Minimum one year of clinical nursing experience Strong nursing assessment and observation skills Excellent patient and caregiver communication Ability to recognize and communicate changes in condition Confidence working independently in patient homes Strong organization and time-management skills Valid driver's license and reliable transportation Willingness to travel throughout your assigned territory
Preferred:
