Job Description

JOB SUMMARY
: To coordinate and ensure the infection control needs of the resident's, facility and employees are prevented, managed, and improved upon through monitoring of the regulatory infection control practices of the long-term care setting. To coordinate and ensure the restorative nursing clinical needs of the residents are identified, care planned, monitored, and evaluated in order to assist with maintaining or improving resident's activities of daily living. The Infection Preventionist (IP) is responsible for the infection prevention and control program for the facility. The Restorative Nurse Lead is responsible for creating, monitoring and evaluating the restorative program in conjunction with the therapy department and director of nursing. The Restorative Nurse Lead oversees the certified nursing assistants in the nursing rehab department.
Hours:
72 hours Per Pay Period, Day Shift, every 4th Weekend/Holiday.
RN:
Starting wage is $40.00 per hour.
LPN:
Wage is depending on experience.
INFECTION PREVENTIONIST
Review infection control policies and procedures periodically and revise as needed to conform to current standards of practice or to address specific facility concerns. Identify and communicate information about residents with potentially transmissible infectious agents. Monitor the frequency and nature of infection control surveillance in the facility. Educate staff and train in caring for residents in isolation/precautions. Education staff and train on essential steps to cleaning equipment. Identify the process for choosing agents for performing hand hygiene. Participates and reports on a regular basis on the Infection Prevention and Control Committee, Quality Assurance Performance Improvement Committee.
SURVEILLANCE/MONITORING
Review of microbiology culture and sensitivity report on a regular basis to identify types of organisms causing infections, antibiotic-resistant organisms, and transmission or organisms between residents. Perform surveillance for infections, compile and analyze data, prepare and bring reports to the Interdisciplinary daily meetings as well as the Infection Prevention Committee. Plan and participate in process improvement activities as needed and indicated by data analysis. Evaluate dietary, laundry, housekeeping, maintenance, and nursing techniques to assure they are consistent with recommended infection control practices. Ensures that reusable equipment is appropriately cleaned, disinfected, or reprocessed. Ensure single-use medication vials and other single use items appropriately disposed after every single use. Monitor employee health, identify trends, and report to interdisciplinary team in order to collaborate to prevent facility outbreaks.
ANTIBIOTIC REVIEW
Review of the use of antibiotics. Involve the consultant pharmacist with the oversight by identifying antibiotics prescribed for resistant organisms. Track antibiotic use monthly and complete an antibiogram yearly or as directed by the Medical Director and the QAA Committee. Review findings at Quality Assurance Committee meetings.
PRACTICES
Assure that products and procedures for cleaning and disinfection of surfaces and equipment are appropriate. Assists Dietary Director with managing of food safety, including employee's health and hygiene, and equipment are appropriate. Monitoring of hand hygiene surveillance across facility.
TRAINING
Confirms that initial and ongoing infection control education is completed to help staff comply with infection control practices. Education and training as appropriate when there are policies and procedures to review and/or implement. Task-specific infection prevention and control training is discipline and tasks specific (e.q. insertion of urinary catheters, blood glucose monitoring, etc.) Follow-up competency evaluations identifying staff compliance.
REPORTING
: Report notifiable diseases to the public health department as directed. Report findings to managers and personnel as performance feedback. Communicate with other health care facilities regarding residents who transfer in and out of facility as potential infection diseases arise that may require precautions. Review both prevalence and incidence of infections at the interdisciplinary weekly meetings, monthly Quality Assurance meetings, and infection prevention and control meetings.
DELEGATION
The Infection Preventionist may delegate certain monitoring responsibilities to the nursing unit level. The monitoring of infections and the infectious process is a facility wide responsibility.
RESTORATIVE NURSE LEAD
Lead restorative nursing program, implementing nursing inventions that promote the resident's ability to adapt and adjust to living as independently and safely as possible. Focus on achieving and maintaining optimal physical, mental, and psychosocial functioning. Participates and Leads Restorative Nursing Meetings regularly. Collaborates and supervises Certified Nursing Assistant is in rehab department to implement and achieve resident specific goals. Collaborates with Director of Therapy, as well as PT, OT, and speech on specific resident needs to establish goals. Collaborates with Director of Nursing on specific resident's goals, outcomes, and plan of care. Monitor monthly restorative goals, and report to Director of Nursing, Administrator, COO, Quality RN, and Quality Committees. Member of Skin and Falls Committee. Monitor restorative equipment and identify resident needs. Collaborate with Administrator and Director of Nursing to order resident specific needs according to clinical need in accordance with budget.
CONTINUING EDUCATION
Attend education programs provided by infection control organizations. Collaborate with other infection control professionals. Remain well-informed on current practices regarding infection control and epidemiology. Actively seeks out learning opportunities to promote professional growth. Access to best practice for infection prevention and control. Ongoing access to current federal, state, and local regulations on infection control requirements.
ADDITIONAL JOB DUTIES
Assist with TMA (Trained Medical Assistants) tasks on nursing department floors. Support nursing department team members needs' that work day shift, evening shift and night shift. Learn scheduling system and backup functions to daily scheduling needs of nursing department. Additional responsibilities as determined by owners, supporting resident care, and operations of St. Otto's Mission, Vision and Values.
