Job Description

Skilled Nursing (RN/LPN) Skilled Nursing Visits for clients in Rogers, St. Paul, and surrounding areas Must have at least 1-2 days of consistent availability (4-8 hours per week). Flexible scheduling available.
Duties:
Provide skilled nursing care in clients' homes in accordance with physician orders and individualized care plans. Perform comprehensive nursing assessments and monitor clients' health status. Administer medications, injections, and prescribed treatments. Provide wound care, catheter care, ostomy care, and other skilled nursing procedures. Monitor and document vital signs, symptoms, and changes in condition. Educate clients and family members on disease management, medications, and ongoing care needs. Coordinate care with physicians, therapists, caregivers, and other members of the healthcare team. Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards. Respond appropriately to changes in client condition and communicate concerns to the appropriate healthcare providers.
Skills:
Current RN or LPN license in the State of Minnesota (or applicable state license). Current CPR/BLS certification. Experience in home health, skilled nursing, long-term care, rehabilitation, hospice, or acute care preferred. Knowledge of nursing assessments, medication administration, and skilled clinical procedures. Strong clinical judgment and critical thinking skills. Excellent communication and interpersonal skills. Compassionate, dependable, and patient-focused approach to care. Ability to work independently while managing multiple priorities. Reliable transportation and a valid driver's license preferred.
