Would you enjoy working with a small caseload of just 6 people? Do you have a passion for helping others and providing quality, person-centered care? AXIS is looking for a compassionate and dependable LPN or RN to oversee health services for adults with intellectual and physical disabilities in a residential group home setting. This is a hands-on role where you will work along side DSPs and provide nursing care while also supporting residents with their daily living needs, independence, and overall well-being. What You'll Do Administer medications and provide treatments and nursing care. Support specialized medical needs, including G-tubes, catheters, and other equipment as applicable. Assist with activities of daily living, personal hygiene, transfers, and daily routines. Advocate for residents and promote their health, safety, dignity, and quality of life. Monitor changes in health and follow up on abnormal labs, provider orders, referrals, and medication changes. Communicate with physicians, on-call RNs, families, and other healthcare professionals. Assist with staff training related to medications, treatments, and medical equipment. Coordinate medical supplies and equipment with the House Supervisor. Participate in monthly house meetings and support infection control procedures. Maintain accurate and timely medical documentation. Job Requirements- current LPN or RN license, prefer someone with a minimum of 1 year experience working with individuals with developmental disabilities, documentation of freedom of TB, valid driver's license and approved driving record, ability to lift up to 50 pounds, pass background study. Open locations/positions : starting pay is $27.60, with an additional $5.00/hr differential pay for overnight position. Overnight positions pay is $32.60 which includes a $5/hr differential pay White Bear Township, Full time 36 hrs/week- every Monday, Tuesday, and Wednesday overnight 8pm-8am $32.60/hr White Bear Township, Full time 36 hrs/week- Every Thursday, Friday, and every other weekend overnight 8pm-8am $32.60/hr White Bear Township, Part time 24 hrs/ every 2 weeks- Every other Saturday and Sunday overnight 8pm-8am $32.60/hr Duties would include providing direct cares, full benefits available.
