Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews to develop personalized care plans. Administer medications, injections, IV infusions, and perform wound suturing following aseptic techniques and safety protocols. Manage complex medical needs such as catheterizations, ventilator care, dialysis support, ostomy care, and tube feeding in home settings. Provide education on medication management, disease processes like Alzheimer's or dementia care, and health coaching tailored to individual needs. Coordinate discharge planning and case management for patients transitioning between hospital stays, nursing homes, assisted living facilities, or outpatient settings. Document all patient interactions accurately using EMR (Electronic Medical Records) systems such as Epic or Cerner while maintaining HIPAA compliance. Respond promptly to urgent situations through triage protocols and critical care procedures; collaborate with physicians on acute pain management and infectious disease care. Skills Proven experience in diverse clinical settings including hospital medicine, ICU, emergency room, trauma centers (Level I & II), or post-acute care environments.
