Job Description

Overview Join our dedicated home health care team as an RN/LPN Nurse, where your expertise will make a meaningful difference in patients' lives by providing compassionate, comprehensive nursing care in the comfort of their homes. This role offers an energizing opportunity to work independently while collaborating with a multidisciplinary team to promote health, safety, and well-being. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews to develop personalized care plans. Administer medications, injections, IV infusions, and perform wound suturing following aseptic techniques and safety protocols. Manage complex medical needs such as catheterizations, ventilator care, dialysis support, ostomy care, and tube feeding in home settings. Provide education on medication management, disease processes like Alzheimer's or dementia care, and health coaching tailored to individual needs. Coordinate discharge planning and case management for patients transitioning between hospital stays, nursing homes, assisted living facilities, or outpatient settings. Document all patient interactions accurately using EMR (Electronic Medical Records) systems such as Epic or Cerner while maintaining HIPAA compliance. Respond promptly to urgent situations through triage protocols and critical care procedures; collaborate with physicians on acute pain management and infectious disease care. Skills Proven experience in diverse clinical settings including hospital medicine, ICU, emergency room, trauma centers (Level I & II), or post-acute care environments. Strong proficiency with EMR/EHR systems like Epic or Cerner; familiarity with
ICD-10/ICD-9
coding and medical documentation standards. Expertise in specialized areas such as cardiology (cardiac catheterization), ophthalmology (Botulinum toxin treatment), behavioral health, geriatrics, pediatrics (neonatal ventilator), or occupational health. Skilled in performing laboratory procedures including specimen collection/processing and phlebotomy; knowledgeable about laboratory experience and diagnostic evaluation. Ability to manage complex cases involving trauma medicine, infectious disease control, hospice & palliative medicine, or working with individuals with developmental disabilities. Excellent patient assessment skills with a solid understanding of physiology and anatomy; capable of managing airway management and aseptic techniques effectively. Strong communication skills for teaching patients and families about medication administration, nutrition, discharge instructions, and health promotion strategies.
