The Licensed Practical Nurse (LPN) works collaboratively with internal and external providers to support integrated, client-centered care. This role assists in establishing care pathways for healthcare needs and helps clients access appropriate services and resources. The LPN provides in-person and phone support, problem-solving, education, patient advocacy, and follow-up care coordination. This position also serves as a liaison for psychiatric providers before appointments and during follow-up care. This is a full-time position. Typical hours of work are Monday-Friday, 8am-5pm, or similar hours.

Job Duties:

Attend meetings and collaborate with internal and external providers to support the Integrated Care process. Must possess computer skills for accurate documentation, folder creation, and the ability to organize forms and other data on the computer. Demonstrate strong communication, assessment, and independent decision-making skills. Assist with the nursing phone line, process orders, facilitate medication reconciliation, coordinate client appointments, and perform other nursing-related duties. Provide services in accordance with the mission and values of Woodland Centers.

Requirements:

Must have a valid MN Licensed Practical Nurse license. Valid driver's license and pass a background check.
