LPN Job Responsibilities Licensed practical nurses choose Phoenix Home Care & Hospice for our values and the opportunities we present to our team members. Our LPNs demonstrate compassion and nursing know-how everyday, by: Assisting in the development, review, and revision of a care plan Providing nursing care in accordance with the physician's plan of treatment, and individual plan of care, as authorized by the client's payer source Observing, recording, and reporting reaction to treatment and any changes in client's condition to appropriate personnel and/or physician Providing ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatmen Participating in education programs, quality assessment activities, nursing meetings, and other staff meetings as required LPN Job Requirements LPNs at Phoenix Home Care should meet the following qualifications:

• One year of experience in a home health care setting preferred
• Active LPN license in good standing
• Driver's license and vehicle with current auto insurance
