The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Responsibilities Provide direct patient care by monitoring vital signs, administering medications, and assisting with activities of daily living. Perform clinical assessments and document patient information accurately within electronic health record (EHR) systems. Assist with medical procedures such as catheterization, IV infusion, and wound dressing changes while maintaining aseptic technique. Collaborate with registered nurses, physicians, and interdisciplinary teams to develop and implement individualized care plans. Conduct patient triage and evaluate symptoms to determine urgency and appropriate interventions. Support patients with specialized needs including dementia, Alzheimer's care, memory care, and behavioral health considerations. Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information at all times. Educate patients and their families on medication administration, disease management, and post-discharge care instructions. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience working in diverse healthcare settings such as long-term care facilities, hospitals, skilled nursing facilities, or assisted living communities. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation. Knowledge of medical terminology, anatomy, physiology, and nursing best practices. Experience with IV insertion, medication administration, specimen collection/processing, and basic life support (BLS). Ability to perform physical examinations and vital sign assessments accurately. Strong communication skills to effectively interact with patients of all ages and their families. Additional experience in critical care environments such as ICU or trauma centers is advantageous but not required.
