Job Description

Looking for 7P-7A. 36 hours a week. Shift differential is $2.00 2nd shift $3.00 3rd shift and $5.00 for weekends. Maple Lawn Nursing Home is seeking a Licensed Practical Nurse (LPN) to join our team. Responsibilities Provide direct patient care, including administering medications and treatments as prescribed. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Assist with daily living activities for patients, including personal hygiene and mobility support. Document patient care activities accurately in electronic health records (EHR) systems. Collaborate with registered nurses and physicians to develop and implement individualized care plans. Educate patients and their families about health management and disease prevention. Maintain a clean and safe environment for patients, adhering to infection control protocols. Participate in discharge planning and coordinate follow-up care as needed. Qualifications Valid LPN license in the state of practice. Experience in critical care, long-term care, or acute care settings is preferred. Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), and EMR systems such as Epic or Cerner. Knowledge of patient assessment techniques and basic life support (BLS) certification is required. Experience working with diverse populations, including those with developmental disabilities, dementia, or Alzheimer's care. Strong communication skills to effectively interact with patients, families, and healthcare teams. Ability to perform procedures such as IV insertion, phlebotomy, catheterization, and medication administration. Understanding of geriatric care principles and familiarity with hospice or palliative care practices is a plus. Pay:
$27.00 - $32.00 per hour
