Job Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess a strong background in patient care across various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will be responsible for delivering safe, effective nursing care while supporting the overall well-being of our residents and patients. Responsibilities Provide direct patient and resident care in accordance with physician orders and established nursing protocols. Administer medications, including injections and IV infusions, ensuring proper documentation and adherence to safety standards. Conduct physical examinations, vital sign assessments, and patient assessments to monitor health status. Assist with medical procedures such as catheterization, tube feeding, and sterile dressing changes using aseptic techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans that address specific needs such as dementia care or acute pain management. Maintain accurate medical records using electronic health record (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA regulations. Support patients with activities of daily living, including mobility assistance with Hoyer lifts and other equipment. Participate in discharge planning, case management, and health coaching to promote optimal recovery and health maintenance. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience in diverse healthcare settings such as long-term care facilities, hospitals, or assisted living communities. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth. Experience with specialized care areas such as Alzheimer's/dementia care, hospice, or ICU is highly desirable. Knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to nursing practice. Ability to perform clinical procedures such as venipuncture, phlebotomy, and medication administration accurately. Strong communication skills with the ability to work effectively within multidisciplinary teams.
