What You'll Do:
- Conduct wellness assessments and monitor resident health changes
- Support medication management and perform MAR/TAR audits
- Communicate with families, physicians and care teams
- Monitor incidents, falls and follow-up care
- Maintain documentation and compliance with regulations
- Support infection control and clinical quality initiatives
- Train and support care team members
What You'll Bring:
- Active LPN/LVN license
- 1+ year experience in senior living or clinical setting
- Strong assessment and documentation skills
- Knowledge of medication administration and compliance
