This position will be offered for 3p-11p shift. Our hourly base rate starts at $25, and the second shift offers a 10% shift differential incentative.

LPN DUTIES

(duties include, but not limited to)

• Check schedule
• Verify all CNAs have made it to the wing
• Complete daily assignment sheet, specify special duties, sign sheet
• Assign breaks, ensure there is staff available on the unit when others are on break
• Make necessary adjustments with staffing, resident assignment with call ins
• Make walking rounds
• Get report from off going Nurse
• Provide comprehensive report to CNAs regarding vital signs needed for acute charting, weekly vital signs, residents with appointments, visiting families, therapy treatment, residents going on LOAs, declining condition, significant changes, fall risk residents, wounds, activities, outings, etc.
• Paperwork completed for appointments, dialysis
• Count Narcotics with off going Nurse, sign Narcotic book, report discrepancies
• Medication pass
• Order any meds that are low or out from Pharmacy, check expiration dates
• Daily treatments, report non-healing wounds, new skin issues, declining wounds
• Acute charting, report declining or change in condition
• Check no BM list daily
• Weekly skin assessment (use Skin Inspection Wizard in Wound Assessment Manager) with weekly Nurses Notes (use Nurses Progress Notes in User Defined Assessments in Quality Assurance)
• Check and clear E-links
• New orders
• check for allergies and duplicate medications
• call family, put order in system (AHT)
• document in Nurses Note
• 24 report sheet
• write any new orders, issues, appointments, falls, deaths, significant changes, skin tears, bruises, concerns, resident complaints, etc. on the 24-hour report sheet, Sign the 24-hour report sheet
• Manage the CNAs, assist as needed
• Coordinate with Unit Manager/RN Supervisor
• Ensure adequate care is being provided to the residents
• Keep Medication carts clean, locked when not in use, privacy screen when computer not being used, sharps container not full, Med pass/apple sauce/pudding used to give medications labeled, no expired medications
• Accucheck machines/devices are being cleaned and recalibrated every night, disinfect after every use
• Treatment carts are clean, no expired treatments
• Observe infection control practices in the facility
• Refrigerators on the unit are clean, no expired items (foods, medications) and a documented temperature log
