Job Summary Harmoni Care Solutions is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide In-Home Nursing Respite (IHNR) services to a client in Carthage, MS. This is a part-time position requiring Saturday
- Sunday 8:30am
- 3:30pm , providing skilled nursing support and personal care in the client's home.
Responsibilities Assist client with bathing and personal hygiene Assist with grooming needs Assist with feeding and mealtime support Provide G-tube (gastrostomy button) care, including site inspection for redness/leakage/irritation, formula and water flush administration per physician order, and button patency checks Recognize and immediately report G-tube complications (dislodgement, blockage, leakage, granulation tissue, signs of infection) to supervising RN Support client with Rett syndrome-specific needs, including communication accommodations (non-verbal/assistive communication), positioning and mobility support, and seizure precaution awareness per care plan Administer medications per physician orders and care plan Monitor and document client's health status, vital signs, and any changes in condition Report changes in client condition to supervising RN and/or care team promptly Maintain accurate and timely documentation of services provided Follow the individual's approved care plan and Harmoni Care Solutions' policies and procedures Ensure a safe, clean, and supportive home care environment Communicate professionally with client, family members, and care team Requirements Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Mississippi Current CPR certification (or willingness to obtain) Valid driver's license Valid automobile insurance Reliable transportation to and from Carthage, MS Current TB skin test Current physical Background clearance (via MS Department of Health) Prior home health, private duty, or IDD Waiver experience preferred Strong communication, documentation, and interpersonal skills Compassionate, patient-centered approach to care Schedule Saturday
