Job Overview We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience across various clinical settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a vital role in delivering exceptional nursing services, supporting patient well-being, and ensuring compliance with healthcare standards. Duties Provide direct patient care by administering medications, injections, and treatments in accordance with physician orders and established protocols. Conduct thorough patient assessments, monitor vital signs, and document clinical observations accurately within electronic health record (EHR) systems. Assist with diagnostic evaluations such as specimen collection and processing, and perform basic procedures including catheterization and sterile techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients' physical, emotional, and cognitive needs. Manage emergency situations effectively by utilizing first aid skills, airway management techniques, and critical care procedures when necessary. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information. Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience working in diverse healthcare settings such as long-term care facilities, hospitals, or assisted living communities. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation. Knowledge of medical terminology, anatomy, physiology, and nursing best practices including IV infusion and infection control protocols. Experience working with geriatric populations or individuals with developmental disabilities is highly desirable. Strong communication skills to effectively coordinate with patients, families, and healthcare team members. Ability to handle multiple priorities efficiently while maintaining attention to detail in high-pressure situations.
