Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and comprehensive assessments. Responsibilities Deliver direct patient care by performing physical examinations, vital signs monitoring, and comprehensive assessments to inform treatment plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage complex cases involving acute pain management, dementia care (including Alzheimer's and memory care), and infectious disease treatment. Collaborate with multidisciplinary teams using electronic health record (EHR) systems like Epic or Cerner to document medical records securely while maintaining strict HIPAA compliance. Support discharge planning and coordinate ongoing care for patients in skilled nursing facilities, hospice settings, or home & community care environments. Provide patient education on medication administration, wound care, tube feeding, and other treatments to promote understanding and compliance. Skills Proven experience working with diverse populations such as geriatric patients, individuals with developmental disabilities, or those requiring specialized dementia or Alzheimer's care. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding systems (ICD-9/10). Skilled in performing physical examinations, venipuncture (phlebotomy), catheterization, sterile techniques, and airway management. Familiarity with EMR/EHR systems such as Epic, Athenahealth or eClinicalWorks for accurate documentation and patient tracking. Ability to handle emergency situations confidently with first aid skills and CPR certification; experience in urgent care or emergency medicine is highly valued. Excellent caregiver communication skills coupled with a compassionate approach to patient assessment and client interaction. Knowledge of long-term care regulations, infection control policies, and hospice or palliative care principles to ensure safe practices across all settings. Pay:
$27.00 - $30.00 per hour
