Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team, providing high-quality patient care during full-time shifts from 3:00 PM to 11:00 PM and 11:00 PM to 7:00 AM. This role offers an excellent opportunity for experienced nursing professionals to work in a dynamic environment, delivering essential care across various settings including long-term care, assisted living, and medical-surgical units. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to patient-centered care. Responsibilities Provide comprehensive nursing care to patients, including medication administration, vital signs monitoring, and wound care. Conduct patient assessments and document clinical findings accurately within electronic health record (EHR) systems such as PointClick Care. Assist with diagnostic evaluations, specimen collection, and processing while adhering to infection control protocols. Manage IV infusions, catheterizations, and airway management procedures with precision and safety. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to patient needs. Educate patients and their families about treatment plans, medication regimens, and health maintenance strategies. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and information. Respond promptly to urgent situations, including acute pain management or emergency interventions in line with first aid and CPR standards. Qualifications Valid LPN license in good standing with relevant state licensing boards. Proven experience working in long-term care facilities, hospitals, or assisted living environments. Familiarity with EMR/EHR systems such as PointClick Care is preferred. Knowledge of geriatrics, dementia care—including Alzheimer's and memory care—and infectious disease protocols. Competence in performing physical examinations, medication administration, venipuncture, and sterile techniques. Strong understanding of long-term care regulations and documentation standards. Excellent communication skills with the ability to work effectively within multidisciplinary teams and with diverse patient populations including geriatric patients and individuals with disabilities.
