Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, skilled nursing services in our long-term care facilitity. Responsibilities Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management. Assist with medical procedures such as catheterization, suturing, specimen collection, and sterile processing to support clinical needs. Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or those requiring memory or hospice care. Support emergency and acute care situations by managing airway management, ventilator support, first aid, and acute pain management. Collaborate effectively with multidisciplinary teams on discharge planning, case management, and patient education to promote optimal recovery outcomes. Skills Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal or developmental disabilities. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), and long-term care regulations. Skilled in using EMR/EHR systems such as Epic or Cerner for accurate documentation and patient record management. Competent in performing clinical procedures including venipuncture, phlebotomy, tube feeding, catheter care, and sterile techniques. Excellent communication skills for caregiver interaction and client education while maintaining confidentiality in accordance with HIPAA policies. Ability to work efficiently in fast-paced environments such as ICU settings, emergency rooms (ER), Level I & II trauma centers or surgical units like ORs.
