Job Description
Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join Embracing Hearts Healthcare. Embracing Hearts Healthcare provides services through the MS IDD Waiver, which is a Medicaid waiver for individuals with Intellectual and Developmental Disabilities. We go in home and provide nursing respite services. The ideal candidate will possess a strong background in patient care, with experience in home care. As an LPN, you will play a vital role in providing high-quality nursing care to patients, assisting with their daily needs, and collaborating with healthcare professionals to ensure optimal patient outcomes. Duties Provide direct patient care. Administer medications and Feedings. Conduct patient assessments, monitor vital signs, and document medical histories. Assist in the development and implementation of individualized care plans. Perform tube feeding, and other nursing procedures as needed. Support patients with activities of daily living (ADLs) while promoting independence. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Educate patients and families on health management, medication administration, and disease prevention. Ensure a safe and clean environment for patients by adhering to infection control protocols. Must be able to perform Trach Care Requirements Valid LPN license in the state of Mississippi. Experience in Home care. Strong assessment skills and ability to perform basic life support (BLS). Experience working with individuals with developmental disabilities or dementia care is desirable. Excellent communication skills and ability to work collaboratively within a team environment. Willingness to engage in ongoing professional development to enhance nursing skills.
