09/16/2026 Job Description Job Description The Full Time Licensed Practical Nurse (LPN) plays a vital role in providing quality nursing care. Reporting directly to the Nurse Manager, the LPN is responsible for supporting patient care through medication administration, monitoring vital signs, wound care, and assistance with ADLs. This position emphasizes thorough documentation, patient education, and effective communication to ensure comprehensive care plan implementation and health assessments. Responsibilities Provide direct patient care aligned with care plans Administer medications safely and accurately Monitor and record vital signs regularly Document patient information and nursing activities educate patients on health management and treatments Perform wound care and assist in emergency situations Collaborate with healthcare team to implement care plans Conduct health assessments and communicate observations Required Qualifications Licensed Practical Nurse (LPN) diploma or certificate Valid LPN License Preferred Qualifications Proficiency in patient care and medication administration Skill in vital signs monitoring Current Basic Life Support (BLS) certificiation Familiarity with Electronic Health Records (EHR) Strong communication and teamwork abilities Attention to detail in clinical tasks
