Complete ordered wound treatments and dressing changes. Monitor residents for new or worsening skin concerns. Complete wound measurements and documentation as assigned. Ensure treatments are completed and properly documented on TAR. Report changes in wounds or skin condition to the DON/RN and provider as appropriate. Assist with weekly skin/body audits. Assist residents with established restorative nursing programs. Work with nursing, CNAs, and therapy to carry out restorative recommendations.
