Job Description
Overview Join our dedicated healthcare team as a Private Duty Nurse (LPN, RN, BSN), where you will deliver personalized, high-quality nursing care directly to patients in their homes. This role offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized medical attention, including those with complex health conditions such as dementia, developmental disabilities, or recovering from acute illnesses. As a vital member of our team, you will utilize your clinical expertise and compassionate approach to promote health, safety, and comfort for your patients while working in a dynamic and supportive environment. Duties Provide comprehensive nursing assessments and develop individualized care plans tailored to each patient's needs. Administer medications, including injections and IV infusions, following strict protocols and documentation standards. Monitor vital signs, perform physical examinations, and evaluate patient progress regularly. Manage complex medical procedures such as catheterization, tube feeding, wound suturing, and ventilator care. Perform tracheostomy care and suctioning as needed Provide nebulizer treatments and airway clearance therapies Coordinate with physicians and other healthcare professionals to ensure seamless continuity of care and discharge planning. Maintain accurate medical records using EMR (Electronic Medical Records) systems like Epic or Cerner while adhering to HIPAA confidentiality policies. Educate patients and their families on health management strategies, medication administration, and safety precautions. Educate family and caregivers regarding patient care needs when appropriate And any other need as diagnosed by the doctor Qualifications Valid licensure as an LPN, RN, or BSN with current state certification. Proven experience in critical care settings such as ICU, emergency medicine, trauma centers (Level I or II), or hospital medicine. Strong background in specialized areas including Alzheimer's care, memory care, hospice, palliative care, Tracheostomy, Nebulizer or geriatrics. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
