Job Description

Evening Home Care RN (4 PM-Midnight) About Mountain Valley At Mountain Valley Hospice & Palliative Care, our mission is simple: Every Patient. Every Family. Every Time. We provide compassionate care and support to patients and families facing serious illness, helping them navigate some of life's most challenging moments with dignity, comfort, and respect. Make a Difference When It Matters Most As an Evening Home Care RN, you'll provide skilled nursing care, comfort, and support to hospice patients and families in their homes during evening hours. This role is ideal for an RN who enjoys working independently, building meaningful relationships, and making a real difference in the lives of others.
Schedule:
Monday-Friday, 4:00 PM-Midnight Responsibilities Provide skilled nursing care to hospice patients. Monitor patient conditions and communicate changes to the care team. Administer medications and treatments as ordered. Support patients and families through education and guidance. Document care accurately in the electronic medical record. Collaborate with the interdisciplinary team to ensure exceptional patient care. Qualifications Current RN license in North Carolina. One year of nursing experience preferred. Hospice, home health, or long-term care experience preferred. Strong communication and critical thinking skills. Valid driver's license and reliable transportation. Why Join Mountain Valley?
