Our healthcare professionals are dedicated to offering recovery with independence to our patients. We are currently seeking an experienced:

HOME CARE LPN OFFICE NURSE

Provides direct patient care, to include using a variety of skills and necessary coordination of services. Completes appropriate documentation as required by policy and procedure and regulation. Assists Patient Care Coordinator and/or Operations Manager in day to day branch office tasks to ensure patient care documentation is complete and submitted timely. Assists Patient Care Coordinator and/or Operations Manager in management of patient care supplies. Manages all commercial insurance payers and Medicare replacement plans for continued authorization regarding home care visits and documents authorization in electronic medical record per software system. Coordinates with Operations Manager and Patient Care Coordinator in the management of Face to Face documentation as required by specific payer source.

Job Requirements:

Current LPN licensure within the state of branch location, and successful completion of nursing education from an approved school of nursing. One year of clinical nursing experience, preferably in an acute care medical/surgical setting. One year of Home Care experience preferred. Ability to independently organize work and provide effective health care services to homebound patients. Effective communication skills (oral and written). CPR certified.
