Job Description

LPN assesses patients home enviroment and their needs. LPN develops a Plan of Care (POC) with assistance from the RN for each client. The LPN is responsible for supervision of care in accordance with the client's plan of care. The LPN will travel to the client's homes, hospitals, nursing homes, assisted living facilities and or rehab facilities to enroll new clients The LPN will be responsible for supervision of their assigned home care services. The LPN is available during business hours scheduled and participates in all activities relevant to the services provided.
  • Participates in developing qualifications and assignments of personnel.
  • Assessment of the client prior to implementing a Plan of Care
  • Develops the Plan of Care along with the RN for the client.
  • Follow-up with caregiver and client to make sure the plan of care is implemented.
  • LPN will be held responsible for the 90-day Plan of Care Review.
  • LPN will be held responsible for the 90-day Supervisory visit.
  • LPN will be held responsible for admission and discharge of clients referred to the agency.
  • LPN will provide in-service education and orientation to employees.
Qualifications:
Must be a graduate of an accredited college or university. Must be licensed in the state of North Carolina as a Licensed Practical Nurse Must have minimum one year of working experience in home care or longterm care experience. Knowledge of patient evaluation procedures. Ability to maintain quality, safety, and/ infection control standards.
