Licensed Practical Nurse (LPN) - Home Health Position Summary The Licensed Practical Nurse (LPN) provides direct, hands‑on nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient independence, safety, and comfort while following individualized care plans and agency policies. Essential Duties and Responsibilities Provide skilled nursing care to patients in their homes according to the plan of care Monitor and document vital signs, symptoms, and overall patient condition Administer medications and treatments as prescribed Perform wound care, catheter care, ostomy care, and dressing changes Assist patients with activities of daily living (ADLs) as needed Observe and report changes in patient condition to the supervising RN or physician Provide education to patients and caregivers on disease management, medication use, and safety Maintain accurate, timely clinical documentation in accordance with regulatory requirements Follow infection control, safety, and confidentiality standards Collaborate with interdisciplinary team members to ensure coordinated care Participate in case conferences, in‑service training, and quality improvement activities Qualifications Education and Licensure Graduate of an approved Practical Nursing program Current LPN license in the state of practice CPR certification (BLS) Experience Previous home health or clinical nursing experience preferred Experience with geriatric or chronic care patients a plus Knowledge, Skills, and Abilities Knowledge of basic nursing principles and home health practices Ability to work independently and make appropriate clinical decisions Strong assessment, communication, and documentation skills Compassionate, patient‑centered approach to care Ability to follow care plans and physician orders accurately Basic computer skills for electronic health records Reliable transportation and ability to travel to patient homes Working Conditions Work performed in patients' homes and community settings May require travel, flexible scheduling, evenings, weekends, or holidays Ability to lift, reposition, and assist patients as needed Reporting Relationship Reports to: Registered Nurse / Clinical Supervisor
