Day Center Coordinator LPN -
Full Time Position Summary:
Under the supervision of the Center Administrator, the Day Center Coordinator LPN, in conjunction with the Health Care Coordinator (RN) or Licensed Nurse, is responsible for supervising the Personal Care Attendants (PCAs) and CNA's in the provision of activities of daily living, day center activities, restorative and supportive care of the EveryAge PACE participants. Monitors and provides a safe environment for participants. Looks for ways to improve the quality and effectiveness of systems and processes at the center.

QUALIFICATIONS

The requirements listed below are representative of the knowledge, skill, and/or ability required. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Must have a valid drivers license, proof of insurance and have means of transportation.
Education:
Bachelor's degree preferred. High school diploma required.

Cardiopulmonary Resuscitation (CPR) certification and First Aid Certification required.

Graduate of a school of professional nursing.

Current North Carolina Licensed Practical Nurse (LPN) License.
Experience:
A minimum of one year's experience working with the frail or elderly population.

One year of management experience, preferably in a geriatric care setting required.
