Job Overview This is a full time midshift position on a medical-surgical unit from 10 a.m. - 10 p.m. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will help ensure patients receive personalized, safe, and effective nursing services. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Assist with diagnostic evaluations like specimen collection, processing, and medical imaging procedures. Manage acute pain and provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or memory impairments. Support complex care needs including ventilator management, tube feeding, wound suturing, and airway management within hospital or long-term care settings. Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to promote optimal recovery and well-being. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation in accordance with long-term care regulations and clinical confidentiality policies. Skills Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, or home care settings. Skilled in performing physical examinations, patient assessments, first aid procedures, venipuncture (phlebotomy), and sterile techniques. Expertise in managing complex cases involving infectious disease care, hospice services, acute care scenarios, emergency medicine including triage and urgent care. Experience working with individuals with disabilities or developmental challenges; familiarity with dementia care and memory support strategies. Ability to handle high-pressure situations confidently while demonstrating excellent caregiver communication skills. Knowledge of long-term care regulations and experience providing geriatric or pediatric patient care is highly valued.
