Patient Population:

Geriatric 3. Key Responsibilities Provide nursing care to long-term care and skilled nursing patients. Monitor vital signs, intake/output, pain, and changes in patient condition. Administer medications and perform nursing treatments. Perform wound care and manage wound vacs and drains. Care for patients with tracheostomies, feeding tubes, central lines, IVs, and TPN. Provide respiratory support including oxygen, CPAP, and BiPAP. Perform blood glucose monitoring and IV therapy. Assist with admissions, discharges, and patient/family education. Provide catheter, colostomy/ileostomy, and feeding tube care. Monitor patients with cardiac, neurological, orthopedic, endocrine, and other chronic conditions. Communicate changes in patient status to the RN/physician. Follow infection control, safety, and patient-care protocols. 4. Requirements Minimum 2 years of nursing experience required. Previous nursing home or skilled nursing facility experience preferred. Active LPN/LVN license in hand at submission. Recent EPIC experience required. Strong long-term care and geriatric nursing skills. Ability to work independently in a fast-paced environment. Willingness to float as required. 5. Education Graduate of an accredited Practical Nursing program. 6. Certifications BLS required Active LPN/LVN license required 7. Preferred Qualifications Previous LTC/SNF experience. Experience caring for geriatric patients. Experience with tracheostomy, wound vac, TPN, central lines, IVs, and G-tubes. Experience with CPAP/BiPAP. Strong documentation and communication skills. EPIC experience.

Pay:

$1,200.00 - $1,400.00 per week
