Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans. Administer medications and injections accurately following physician orders, including IV infusions and catheterizations. Manage acute pain and assist with complex medical procedures such as wound suturing, gastric lavage, or spinal taps. Monitor patients' conditions closely using electronic health record (EHR) systems like Epic or Cerner; document all care activities thoroughly and accurately. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Assist with diagnostic evaluations such as specimen collection/processing and medical imaging coordination when needed. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure compliance with long-term care regulations. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS. Proven experience working in diverse healthcare settings such as hospitals (including ICU or Level I Trauma Centers), nursing homes, or home & community care environments. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with
ICD-9/10
coding is preferred. Experience providing specialized care including IV infusion, ventilator management, tube feeding, or hospice care is highly desirable. Strong knowledge of anatomy, physiology, infection control protocols, and medication administration procedures. Ability to communicate effectively with patients of all ages—including geriatric patients—and collaborate seamlessly within multidisciplinary teams.
