Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Provide education to patients and families regarding health management, medication administration, and post-discharge instructions. Ensure infection control practices are followed rigorously to maintain a safe environment for patients and staff. Experience Proven experience working as an Licensed Practical Nurse in nursing homes, or outpatient clinics. Familiarity with long-term care regulations and geriatric patient needs within skilled nursing facilities or assisted living communities.
