Hours Days Shift Details:

6.00 AM to 6.30 PM 36 hours/week. (3

• 12) 13-week contract Administer medications, including injections and IV infusions, following established protocols and safety standards.

Conduct comprehensive patient assessments, monitor vital signs, and document medical information accurately within electronic health record (EHR) systems. Assist residents with activities of daily living, including mobility, hygiene, and nutrition support, ensuring dignity and comfort. Manage wound care, catheterizations, and other specialized procedures with precision and adherence to aseptic techniques. Collaborate with physicians, therapists, and caregivers to develop and implement individualized care plans tailored to residents' needs. Monitor for signs of acute pain or changes in condition, providing timely interventions or triage as necessary. Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and facility protocols to maintain a safe environment for residents and staff. Experience Proven experience working in long-term care facilities such as nursing homes or assisted living communities. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner for accurate medical documentation. Critical care experience or hospital medicine background is highly desirable to handle complex resident needs effectively. Knowledge of dementia care, Alzheimer's disease management, and memory care strategies. Experience working with individuals with developmental disabilities or behavioral health considerations is advantageous. Familiarity with

ICD-9/10

coding for medical records and documentation purposes. Prior experience performing medical procedures such as venipuncture, tube feeding management, or sterile processing is preferred.
