SUMMARY/OBJECTIVE
Provides professional team-based care to patients. Assists in the continuum of care and in educating patients on their patient centered medical home.
ESSENTIAL JOB DUTIES
(This list may not include all of the duties assigned) Performs general nursing care to patients. Screens patients for appropriate information. Administers prescribed medications, tests and treatments in accordance with nursing standards. Utilize nursing knowledge and skills in the safe implementation of basic preventative and therapeutic nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes. Assists with the preparation of equipment and aids physician during treatment, examination, and testing of patients and documents accurately in the patient's chart. Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards. Responds to and refers incoming telephone calls. Instructs patient and family regarding medications and treatment instructions, within scope of practice. Responsible for preparation of exam rooms including re-stocking of treatment areas. Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback. Participate in professional development activities and maintains professional affiliations. Performs related work as required.

EDUCATION AND EXPERIENCE
RN Required:
Graduation from an accredited school of nursing Possession of a
State Registered Nurse License CPR Certification Preferred:
One year of professional nursing experience in a clinic setting preferred Word processing and computer experience
LPN Required:
Graduation from an accredited school of practical nursing. Valid state license to practice practical nursing
CPR Certification Preferred:
Word Processing and computer experience. Job description is available upon request.
