- Complete skilled nursing visits in patients' homes
- Perform patient assessments and ongoing evaluations
- Administer medications and treatments as ordered
- Provide wound care and other skilled nursing services
- Educate patients and family members regarding medications, disease management, and home safety
- Document visits accurately using electronic charting
- Communicate changes in patient condition to physicians and the clinical team
- Coordinate care with other healthcare providers as needed Qualifications
- Current New Hampshire Licensed Practical Nurse (LPN) license required
- Home care, home health, rehabilitation, long-term care, or community nursing experience preferred
- Excellent assessment and communication skills
- Reliable transportation and valid driver's license
- Ability to work independently while managing your own schedule Coverage Area Visits may be available throughout Belmont, Laconia, Gilford, Tilton, Franklin, Meredith, Sanbornton, Gilmanton, and surrounding Lakes Region communities based on your availability and preferred travel area. Sign-On Bonus Eligible nurses will receive a $1,500 sign-on bonus paid in three installments:
- $500 after successful completion of orientation and two weeks of active employment with a minimum caseload
- $500 after three months of continuous employment
- $500 after six months of continuous employment Payments are made through regular payroll and are subject to eligibility requirements and applicable taxes.
