$2,342 Position Overview We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join the Long-Term Care team at Sullivan County Health Care in Claremont, New Hampshire. This is an excellent opportunity for an experienced LPN who is passionate about providing quality, patient-centered care to residents in a supportive LTC environment. Key Responsibilities Provide safe, compassionate nursing care to long-term care residents. Administer medications and treatments according to established protocols. Monitor residents' conditions and promptly report changes. Perform assessments and maintain accurate clinical documentation. Collaborate with RNs, physicians, CNAs, and other members of the healthcare team. Assist with activities of daily living and promote resident comfort and dignity. Follow infection-control, safety, and facility policies. Build positive relationships with residents and their families. Ideal Candidate Active LPN license Previous Long-Term Care/SNF experience preferred . Strong clinical assessment and communication skills. Reliable, compassionate, and team-oriented. Ability to work evening shifts and commit to a 48-hour weekly schedule .
