Licensed Practical Nurse LPN - St. Johnsbury, VT We are seeking a skilled and compassionate Licensed Practical Nurse LPN to join our team at St Johnsbury Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider. Role:
As a Licensed Practical Nurse LPN, you will be responsible for providing high-quality patient care and contributing to the overall well-being of our residents.
Your daily responsibilities will include:
Assessing and evaluating patient needs and developing individualized care plans Administering medications, treatments, and therapies as prescribed by physicians Monitoring patient vital signs and reporting any changes to physicians or other healthcare professionals Collaborating with other healthcare team members to ensure seamless care transitions Providing emotional support and education to patients and their families
Responsibilities:
Provide direct patient care and observation in accordance with established nursing standards and policies Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals Conduct thorough patient assessments and document findings in accordance with facility policies Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement Maintain accurate and up-to-date patient records and reports
Requirements:
Current LPN licensure in the state of Vermont High school diploma or equivalent required; associate's or bachelor's degree in nursing or a related field preferred At least [insert minimum years of experience] years of experience as an LPN; [insert level of experience] experience in a long-term care or rehabilitation setting preferred
