Skilled Nursing Facility (SNF) Job Summary We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) for a 14-week contract assignment at a Skilled Nursing Facility in Concord, NH . The LPN will provide direct nursing care to residents, administer medications and treatments, monitor patient conditions, and collaborate with the interdisciplinary care team to ensure high-quality resident care. Responsibilities Provide direct nursing care to residents in accordance with individualized care plans. Administer medications and treatments as ordered and document appropriately. Monitor residents' health status and promptly report changes to the RN or provider. Perform nursing assessments and maintain accurate, timely clinical documentation. Assist with wound care, treatments, and other skilled nursing procedures as appropriate. Communicate effectively with residents, families, physicians, RNs, CNAs, and other members of the healthcare team. Support activities of daily living and promote resident comfort, dignity, and safety. Follow facility policies, infection-control procedures, and applicable state and federal regulations. Respond appropriately to changes in resident condition and emergencies. Maintain accurate medical records and shift reports. Requirements Current, active LPN license in the State of New Hampshire or eligibility to practice in New Hampshire. Previous SNF/LTC experience preferred . Current CPR/BLS certification preferred or required per facility policy. Strong clinical assessment, communication, and organizational skills. Ability to work independently and as part of a multidisciplinary team. Commitment to providing safe, compassionate, and resident-centered care.
Pay:
$36.00
•$45.00 per hour Expected hours: 40.0 per week
