Job Description
RiverWoods Exeter is seeking compassionate RN/LPN nurses to join our Healthcare & Medical Services team in our continuing care retirement community. In this resident-focused role, you'll provide direct nursing care, administer medications and treatments, coordinate with interdisciplinary teams, and support care plans across independent living, assisted living, and nursing care settings. Responsibilities Provide direct nursing care to residents in independent living, assisted living, and nursing care settings Administer medications, treatments, and injections according to physician orders and facility protocols Monitor resident conditions, document assessments, and update electronic health records accurately Develop, implement, and evaluate individualized care plans in collaboration with the care team Communicate changes in resident status to physicians, families, and interdisciplinary team members Assist with admissions, transfers, and discharges, ensuring smooth transitions of care Supervise and support LPNs (for RNs), LNAs, and other caregiving staff, providing guidance and oversight Promote resident safety, dignity, and comfort while following infection control and regulatory standards Participate in care conferences, quality improvement initiatives, and staff education Educate residents and families on health conditions, medications, and wellness practices Required Skills Medication administration Wound care IV therapy (if scope of practice) Electronic Health Record (EHR) documentation Care planning and clinical assessment Geriatric/long-term care nursing Infection control and prevention Vital signs monitoring and interpretation Patient and family education Interdisciplinary team collaboration
