Part-Time/PRN APP:

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Maintain timely and accurate documentation in ChartEasy™
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements Employee must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
